Office Policies
If you are interested in becoming a patient at COC please call our office, 614-785-1115 option 2 to request a link to our patient portal where you will complete our required paperwork. If you have questions about the services we offer please call us at 614-785-1115, option 2. It is the patient’s responsibility to contact his/her insurance company to verify in-network status for their provider, copay and deductible information. Please note that we do not provide disability assessments, nor will our providers complete paperwork for disability services.
Appointment Scheduling
In order to schedule an appointment you will need to contact our office at 614-785-1115 option 2 to request a link to our Patient Portal.
Once you have set up your patient portal account, forms 1 & 2 (see "Patients Forms" section below) will need to be completed. These forms are submittable through our portal site. We will also need you to email us the front & back of your insurance card(s) and your photo ID. The simplest way to do this is to take pictures with a smart phone and email them to us at frontdesk1@centralohiocounseling.com. You may also stop by our office to complete your paper work. Once we have all the necessary paperwork & card copies our Intake Scheduler will call you to set up an appointment.
Late Cancellation / Missed Appointment Policy
COC requires a 24 hour notice if you are cancelling your appointment. If you fail to contact our office at least 24 hours prior to your scheduled appointment or if you do not show for your scheduled appointment we will charge you a $60 fee for the late cancellation or no show.
Prescription Refill Policy
Your prescriptions will be filled at your regularly scheduled appointment. If you find that you need refills prior to your appointment your doctor may charge a $25 refill fee for these prescriptions. Please note that it is at your physicians discretion as to whether he/she will refill controlled medications. In most instances you will need to be seen for an appointment for these medications to be refilled. Also, please note you will need to contact our office to request any refills as we do not honor faxed prescription requests from your pharmacy. Please allow 2 business days for refills to be processed. We will not process refill requests after 12pm on Friday or over the weekend.
Existing patients that are in need of a medication refill please be advised that you will need to call our office and schedule an office visit with your provider.
Medical Records Requests
If you are requesting medical records please click on the Release of Information link to download a release to send medical records. Please fill out the release in its entirety and fax to our office at 614-785-0095. If you are requesting records be forwarded to another physician we will be happy to send them via fax at no charge. If you are requesting a paper copy of medical records for personal use we reserve the right to charge a reasonable fee for time and supplies. Please allow 7 business days for medical records requests to be processed.
Disability Forms
COC does not provide disability assessments nor will our providers fill out paperwork for disability claims. If you are currently on disability or seeking disability please be advised of this policy.
Patient Forms
In order to become a new patient you will need to fill out & submit Packets #1 and #2.
You will also need to email us a copy of your photo ID & front/back of your insurance card to newpatient@centralohiocounseling.com.
Once we've received all of your information we will call you to set up your first appointment.

Emergencies
If you are in crisis please go to the nearest emergency room or contact Netcare Access at 614-276-2273 or call 988 for the national suicide/crisis hotline.
